Sources had previously told The War Zone that the incident occurred when a tablet computer was ingested in one of the engines, which caused substantial damage, necessitating the replacement of both engines on one side of the aircraft. The 7th Bomb Wing would neither confirm nor deny any of these specific details.

Only a couple of engines! Have some pity for the maintainers of KC-135 (57-1418 at Tinker AFB) who didn't notice that during a ground pressurisation check the hull-pressure gauge passed zero for a second rotation... & blew the jet in half! :roll: :cool:

The (female) maintenance commander at Dyess has been relieved of command - as you many here that work in safety critical environments know, individuals don't cause accidents - poor culture does. Nothing dangerous ever has just a single safeguard protecting those around it from an accident, multiple things have to go wrong in safety critical environments for accidents to happen. According to t'other forum, there were already 'multiple maintenance-related safety investigations occurring' before this incident. The B-1 is the most maintenance intensive aircraft in the USAF, apparently, but it seems that allowed safety to be overlooked.
